Budget: $8 - $15/hr
Each month we reach out to a panel of patients under the Principal Care Management (PCM) program and we now need a clinically trained contractor who can take over that workflow from start to finish. You must have a degree or formal training in Pharmacy, Nursing, or a closely related clinical field and feel completely comfortable discussing opioids, benzodiazepines, other narcotics, and their potential interactions in clear, professional English.
Scope of work • Place structured, HIPAA-compliant telehealth calls, focusing on routine check-ups that meet PCM timing and content requirements. • Complete a full medication reconciliation on every call, flagging high-risk agents. • Chart each encounter in SOAP format, with time-stamped, audit-ready accuracy inside our EHR. • Escalate any of the following issues to the prescribing provider on the same day:
- Non-adherence
- Adverse effects
- Therapy gaps • Coordinate smoothly with our interdisciplinary care team so recommendations are actioned and documented.
What we're looking for You handle sensitive clinical conversations with empathy yet authority, keep immaculate records, and already understand pharmacology well enough to spot red flags without a script. Telehealth experience is welcome but secondary to your clinical foundation and documentation skills.
Deliverable acceptance criteria
- All assigned patients contacted within the calendar month.
- 100 % of notes entered in correct SOAP structure.
- Escalations delivered via the designated template within 24 hours.
- Zero HIPAA breaches.
