Interactive Fillable PDF With Scoring

I already have the layout of my form sketched out; what I need now is for you to turn it into a truly interactive PDF. Every checkbox must be fully fillable so the end-user can tick boxes directly on screen.

At the bottom of the document a live calculation has to run: instead of a simple sum, the form should generate a weighted score driven by the frequency of selections made across the entire form. In other words, the more often an option is chosen, the more it should influence the final score. This logic needs to update instantly as boxes are checked or cleared.

Please use Adobe Acrobat (or an equivalent tool that supports embedded JavaScript) so the mathematics happen inside the PDF itself—no external spreadsheets or web calls.
