Case Manager (one) Job Summary The Case Manager will be responsible for case identification, case planning, follow-up, referral, and documentation to ensure beneficiaries in Dire Dawa receive appropriate and timely support under the project. Key Responsibilities Register and assess project beneficiaries Prepare individual case plans Provide regular follow-up and case monitoring Refer beneficiaries to training, health, legal, and social services Maintain case files and database Support monitoring and reporting activities Coordinate with partners and service providers
