In this role, you'll work at the heart of local communities-connecting with people who are facing challenges and helping them break down barriers so they can live safe, healthy, and fulfilling lives.

  • Working alongside existing community groups and networks to identify individuals and families who would benefit from support.
  • Forming and maintaining relationships with individuals and families who may face a variety of challenges.
  • Co-producing plans of support with individuals, ensuring that these are meaningful to them.
  • Maintaining records on electronic systems.
  • Compiling monthly statistical returns.
