I am currently representing Cheshire East Council are offering an initial temporary contract for a period of 4 months with a view to be extended or offered a permanent position for the right candidate at a rate which is negotiable dependent upon experience.

We are looking for Social Care Assessor at Leighton Hospital, CW1 4QJ

Within the framework of legislation, agreed Council Policies and Procedures, to work with and on behalf of vulnerable people by completing the required needs based assessments, arranging a full range of services and care managing a case load of clients and carers.

Key Accountabilities:

Carry out basic needs-based assessments with vulnerable people and their carers within agreed procedures, to determine specific individual client needs, identify levels of risk and urgency of response to construct an individual Care Plan.

  • Implement, co-ordinate and maintain individual Care Plans and act as Care Manager to ensure that services are arranged to meet the identified needs of vulnerable people.
  • Monitor, evaluate and review individual Care Plans, identify, and respond to significant changes by modifying the Care Plan to ensure that the identified needs continue to be met in the most cost-effective way.
  • Negotiate and liaise with other professionals, statutory and voluntary agencies, relatives, and carers about care arrangements in order to maintain the best possible service provision for clients and carers.
  • Inform clients and carers of the financial implications of care services, undertake financial assessments, and encourage clients to access benefits advice in order to maximize income to clients, carers and the department.
  • Prepare and maintain client records/systems, both written and electronic to ensure the provision of accurate up to date client related information to the group

Requirements :

NVQ 3, or equivalent, in Social Care

  • Experience of working with vulnerable people and their careers in a Social Care or Health setting
  • A willingness to develop and adapt to new Initiatives
  • Working knowledge of and implications of relevant Acts and care standards
  • Understanding of social work theory and models of intervention which underpin work
  • Knowledge of assessment and care management process, including how to undertake needs-based assessments
  • General awareness of mental health and/or general health issues Broad knowledge of other disciplines e.g. occupational therapy, speech therapy, etc.
  • Understanding of statutory and policy requirements
  • Ability to risk assess and identify needs and resources to meet identified needs Clear understanding of risk and safeguarding procedures
  • The ability to relate to vulnerable people
  • Experience of care Management
  • Ability to communicate effectively
  • Ability to work in a multidisciplinary setting and develop integrated approach Good IT skills - ability to use a variety of computer systems and good keyboard skills
