Database design for our business

Hi can anyone help me with a simple database for our nursing home.
Requirements are as follows:
To have a contact and information list for all our current clients and the ability to link each resident to some kind of task XXXX XXXX table
The task XXXX XXXX would take each resident through the admission process tasks all the way up to resident discharge tasks (these would be different for each resident based on what their funding status is) where we could update on task XXXX XXXX see at a glance what is completed and what is still pending.
Needs to be (subscription free ideally?) cloud based and allow multiple user access at the same time
Would also ideally need to be able to add attachments as necessary to a task XXXX XXXX if you have done something similar and have examples you can show.
Project posted at upper limit of budget - bids coming in lower will be considered more favourably!
