Small webpage

I am creating a very small web page to go with my YouTube channel. I would like the web page to include a form so that people can send me story ideas anonymously. I already have a logo and background for you to use. I would also like my most recent YouTube videos to automatically load onto the web page if possible. I need somebody who can develop this page for me and put it up on my domain name and hosting that I already have. Budget: USD 30–250 Skills: Digital Design, Visual Design, Web Design
