Personnel Allocation Supervisors: One per Woreda; total 14 Team Composition: Each woreda will be staffed by one person (supervisor and 5 data collectors) Duration of Assignment Supervisors: 12 days total, inclusive of travel. Data Collectors: Duration aligned with supervisor’s schedule. Transportation MERQ will arrange vehicles, if vehicles are unavailable, transportation allowance will be provided. Training and Preparation Supervisors:3–4 days Training of Trainers (TOT) in Addis Ababa. 2–3 days woreda-level training prior to data collection. Roles and Responsibilities Organize and oversee fieldwork; ensure adherence to protocols Provide guidance and troubleshoot challenges Validate completeness and accuracy; monitor ethical compliance Submit daily progress updates and woreda-level summary reports Conduct qualitative data collection at woreda  Conduct facility assessment  Review the protocol, understand the purpose of the data collection and internalize the quantitative and qualitative data collection tools Avail to receive team standardization training and participate actively in all sections of the training Organize and provide training at woreda level for data collector  On daily basis, assess the progress of the team and revise plan as needed to complete the data collection within the planned schedule. Make the necessary arrangements for the fieldwork. Prepare and submit the team’s data collection plan to MERQ that shows the timing and plan for collecting data from each study site. Organize and direct the fieldwork. Ensure field staff are performing their task XXXX XXXX the guidance and procedures provided in the training. Ensure field staff complete their responsibilities. Ensure all data collection tasks are completed daily. Submit all completed questionnaires to the server on a daily or every other day basis. Serve as a link between the enumerator and study core team members of MERQ. Provide regular data collection progress updates to MERQ. Coordinate and ensure the appropriate use of field logistics, including vehicles and fuel; Conduct quality check on the collected data, on daily basis whenever possible; Receive feedback from the central office (from the data manager, the central Data quality assurance team, project coordinator, etc.) and make a correction or justification as needed. Ensure that the team collects data with the required quality (completeness, reliability, accuracy, integrity, precision, confidentiality) Plan the activities for the day to maximize efficiency and effectiveness Submit all data collection materials to MERQ at the end of data collection. Carry out any relevant duties and responsibilities assigned the central team in relation to the field work. Reporting Lines Supervisors report directly to MERQ project coordinators. Data collectors report to their assigned supervisor. Supervisors consolidate woreda-level reports and submit to MERQ.
